How to Complete & Submit Your Registration
Follow these 4 simple steps to complete your child's enrollment.
Download & Fill
Download the PDF form above for your child's age group. You can type into supported PDF form fields using a compatible PDF reader, or print the form and write clearly. Review and sign all required sections; some signatures may need to be handwritten.
Gather Documents
Include a copy of your child's immunization record, emergency contacts, and provincial Childcare Subsidy approval number (if applicable).
Send or Drop Off
For sensitive child or family information, we recommend delivering completed forms in person. Call (780) 752-2525 to confirm an approved secure electronic submission method.
Confirm & Start
Our director will review your submission, confirm room availability, and contact you to schedule an orientation before your child's start date.
Registration Checklist
Please review your package before sending it to the daycare.
- Complete all sections that apply to your child.
- Sign and date every required consent section.
- Confirm emergency contacts, phone numbers, and email addresses.
- Attach immunization records or other requested documents.
- Save a copy of the completed form for your records.

Can I fill out the PDF digitally on my computer or phone?
Both PDFs include fillable fields. Download and open the file in a compatible PDF reader such as Adobe Acrobat Reader; save a completed copy and reopen it to check your entries. Electronic signatures are not guaranteed to be supported for every signature field, so print and sign where required. Are Childcare Subsidies and Affordability Grants accepted?
Yes, Southgate Daycare is fully licensed under Alberta Early Learning and Child Care. We accept the provincial Affordability Grant (applied directly to reduce fees) as well as the Alberta Child Care Subsidy for qualifying families.
What happens after I submit the registration form?
Once we receive your completed package, our program supervisor will review your information, check group availability for your requested start date, and contact you by phone or email. If you do not receive confirmation within two business days, please call (780) 752-2525.
